The Community Liaison is responsible for building and maintaining customer relationships for referral partners in their assigned territory.
The Marketing Liaison will participate in community education activities and engage in community outreach efforts.
As a Community Liaison you may be asked to educate the community, families and referral sources about hospice services.
Be knowledgeable in completing hospice consents with patients and or family.
The Marketing Liaison is responsible for providing customer service to ensure performance meets expectations and to increase referral volume and education in the communities Care Comfort Hospice serves.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following:
Markets the services and programs of Hospice to key referral sources including physicians, nursing homes, assisted living facilities and hospital personnel including but not limited to case managers, discharge planners and other appropriate referral sources.
Develops and manages accounts, facilitates dialogue between the Care Comfort Hospice staff and referral partners to ensure seamless care delivery and communication from initial referral through each patient’s discharge.
Meets with patients prior to discharges from hospitals or nursing homes to coordinate care and define resources to ensure safe transition to home.
Obtains pertinent demographic and medical information for written referrals to ensure appropriate care can be initiated upon patient discharge.
Assists with delivering orders for signature to physician offices.
Develops and conducts educational programs and related printed material.
Participates in community education and outreach efforts.
Monitors and reports any feedback from referral sources, communities, patients or customers that may positively or negatively affect agency operations.
Actively participates in quality assurance and performance improvement (QAPI) teams and activities.
Works independently but willingly assists other team members as necessary to accomplish company goals, customer service and client needs as required.
Maintains accurate expense reports and accounting for market activities.
Willingly accepts other duties as assigned.
Knowledge of:
Care Comfort Hospice services and philosophy
Insurance Reimbursement including Medicare and Medicaid
Assessment protocol and admission criteria for Care Comfort Hospice services
Microsoft Office software packages, Internet, Email

Requirements
QUALIFICATIONS FOR EMPLOYMENT
Education: Associate’s or Bachelor’s degree in a healthcare related field preferred.
Experience: Two (2) years recent marketing/sales experience preferred.
Two (2) years recent healthcare experience required with preferably one (1) year experience in Hospice Care.
Previous clinical experience preferred.
Valid Pa.
driver’s license with an automobile that is insured in accordance with state and/or agency requirements and is in good working order.
